if you have slow accumulation, large volumes of effusion might be tolerated

effusion is surrounding the heart

You gotta watch out for the catastrophes
drain if you have suspicion of tamponade or suspicion of infection
usually drain a little bit for testing if you are worried about malignancy or infection
fluid due to heart failure, radiation therapy will be just transudate, not exudate or blood
cardiac silhouette shrinks down after steroid administration
