  • central or proximal parts of acini, primary respiratory bronchioles, are affected

  • lesions tend to be more severe in upper lobes

  • walls of the emphysematous spaces often contain large amounts of black pigment

  • centrilobular emphysema is diagramatically different from panacinar emphysema

  • grossly: stretched-out respiratory bronchioles, but around the stretched-out bronchioles, the parenchyma looks okay—not so stretched out
