  • most common cause of thyroid pain

  • accounts for up to 5% of patients with clinical thyroid disease

  • usually, middle-aged women

  • viral infection or post-inflammatory process?

    • associated with a prior viral infection

    • inc. coxsackievirus, mumps, measles, adenovirus

  • usually hypothyroidism

  • may be preceded by a period of hyperthyroidism

pathology

  • rupture of follicles with inflammation

  • formation of granulomas

  • thyroid is enlarged 2-3x normal size

  • tissue injury does not seem to be self-perpetuating and is self-limited

granulomatous

  • you see multinucleate giant cells
