a) Congenital : a supernumerary extra finger, cleft palate.
b) Pathological: asymmetry of bones as in poliomyelitis. Long mastoid on one side may be and evidence of torticollis.
c) Traumatic: mal-united or non-united old fracture, fixation of fracture with plates or intramedullary nails.
d) Occupational: depression of the sternum in shoe-makers
