Dental consequences of GERD

Nocturnal gastric acid reflux results in significant acidic gastric fluid entering the oral cavity. Patients complain of a bitter, acid taste in their mouth. Chronically, this acidic environment results in significant enamel erosion and rampant caries.

Pharmacological Mgt of GERD:

  1. acid suppression (best: proton pump inhibitors)
  2. alter gastric-esophageal dynamics (e.g., metoclopramide [Reglan])
    1. increase esophageal peristalsis
    2. increase esophageal sphincter tone
    3. promote gastric emptying
  3. antacids mask the symptoms of dyspepsia
