Nitrates are effective because they decrease oxygen demand. The principle tissue mechanism of action is a reduction in pre-load to the heart. antagonists decrease heart rate, systemic BP and cardiac contractile activity—good in one way, bad in another. Calcium antagonists reduce preload by the same mechanism as nitrates, and increase coronary flow. Aspirin inhibits platelets and cyclooxygenase and reduces the incidence of coronary thrombosis. If drugs don’t work, the angina must be treated by bypass surgery.
