Cerebral infarct morphology

  • After first 12 hours see red dead neurons

  • Up to 48 hours see neutrophils

  • Within days the neutrophils are replaced by macrophages stuffed with neuronal debris and may have hemosiderin in them if the infarct was hemorrhagic

  • 1 – 2 weeks see gliosis (not fibrosis)

  • May resolve as a cystic cavity
