example 1: overdrive suppression of sinus node by atrial fibrillation

  • in these patients, when atrial fibrillation stops, sinus node recovery can be prolonged

  • slow recovery of the sinus node is indicative of sick sinus syndrome, which can require a permanent pacemaker


example 2: atrial fibrillation in Wolff-Parkinson-White syndrome (WPW)

  • in WPW, you have a fast accessory conduction tract to allow AP to move from atria to ventricles very quickly

  • this leads to a wide-complex tachycardia that is irregular with intermittent pauses

  • rx: procainamide to slow conduction through accessory pathway
