Wiggers of constrictive pericarditis

  • y descent happens during the last part of ventricular filling. this disappears in cardiac tamponade

  • in constrictive pericarditis, all the filling occurs quickly, so you get a very steep y descent

  • on JVP exam, you notice the neck veins collapsing quickly rather than bounding up

  • try to elicit a Kussmaul’s sign. when the patient takes a breath in, this would normally bring more blood into the heart. but you have a shell, so the neck veins don’t empty. in fact, sometimes they even bulge out

  • can also happen if the RV itself fails
