clinical definition: cough with sputum for at least 3 months and in at least 2 consecutive years
however, if emphysema is associated, its extent cannot be determined during life
histology: hyperplasia of mucous glands in bronchi
Reid index: 0.4 is normal. 0.7 or greater means severe bronchitis
COPD may be caused by chronic bronchitis alone or a combination of bronchitis and emphysema
