Surgery

  • thyroidectomy is near-complete

  • remove palpable lymph nodes (central compartment)

  • no radial neck dissection


131I

  • identify tissue by scanning dose (2-3 mCi)

  • high thyroglobulin level is sometimes an indication for treatment

  • treatment dose 100-200 mCi 131I

    • at least 6 months between treatments


Thyroxin replacement

  • T4 100-200 ug/day

  • keep TSH 0.3-1 uIU/ml

  • keep thyroglobulin below 0.5 ng/ml


Radiation therapy: bone metastasis, medullary CA, obstructive disease in neck

Chemotherapy: anaplastic CA, medullary CA
