  • diagnosis

    • acute phase: ↑FT4

    • late phase: ↓FT4

    • 123I uptake is very low in these patients

    • red cell sedimentation rate↑

    • elevated interleukins

  • therapy

    • initial phase: antiinflammatory: aspirin, ibuprofen, prednisone

    • ipodate: inhibit T4 T3 conversion

    • hypothyroid phase: T4 replacement

    • usually transient, so you can slowly reduce the dose


Postpartum thyroiditis

  • second phase is particularly devastating

  • 25% become permanently hypothyroid
