The subendocardial zone has the least blood supply. The inner 1/3-1/2 of the ventricular wall is infracted, as opposed to transmural infarcts. Conditions that cause this type of unusual infarcts:
‘triple vessel disease’ + fall in CO (global hypotension)
normal coronary arteries but fall in CO 2nd to a) bypass surgery, b) prolonged hypoxaemia, c) head injury
congenital: aortic coarctation, supra-valvular aortic stenosis
