due to thrombus or coronary vasospasm occluding vessel (coronary artery injury) -> acute MI

O2 supply greatly decreased independent of myocardial demand

• rise in ST segment


may be due to rise in catecholamines

or increased number / affinity of alpha receptors in large coronary vessels

Ergonovine maleate (weak -agonist) is used in diagnosis by inducing coronary spasm

NE, Epi (-agonist), and methacholine (parasym. agonist) also induce variant attacks

Beta blockers may aggregate the condition too

Treatment: dilate coronary artery
