•angina at rest or early morning hours
•due to thrombus or coronary vasospasm occluding vessel (coronary artery injury) -> acute MI
•O2 supply greatly decreased independent of myocardial demand
• rise in ST segment
•may be due to rise in catecholamines
•or increased number / affinity of alpha receptors in large coronary vessels
•Ergonovine maleate (weak -agonist) is used in diagnosis by inducing coronary spasm
•NE, Epi (-agonist), and methacholine (parasym. agonist) also induce variant attacks
•Beta blockers may aggregate the condition too
•Treatment: dilate coronary artery
