Eczematous plaques on the bilateral lower legs/ankles accompanied by purple, hyperpigmented edematous skin. The skin may be itchy. Involved skin may easily ulcerate, heal slowly and become superinfected.
Diagnosis
Clinical history and appearance.
Pathology
Poor venous return leading to chronic edema & stasis changes.
Principles of Therapy
It is important to minimize leg swelling by combining leg elevation, compression and perhaps medication (diuretics if needed). The eczematous plaques may be treated with moderate potency topical steroids (triamcinolone). Ulcers need good wound care and careful antibiotic therapy when infected. Unfortunately, this tends to be a chronic problem without cure.Category: Dermatology Notes
