Methylation of 3-OH gp on morhine → ↓first pass metabolism
- 10% demethylated to morphine, rest to inactive norcodeine
- maximum analgesia 60mg po = aspirin 650mg
- maximum analgesia 120mg IM = 10mg morphine
- anti tussive. Minimal sedation, resp dep, nausea, constipation
- IV codeine → Histamine
