  • failure of embryologic truncus arteriosus to septate into aorta and pulmonary artery

  • truncal valve may have 2, 3, or 4 cusps

  • subtypes are based on truncus origin of pulmonary arteries

  • VSD is often present

  • early cyanosis with extensive mixing of blood

  • requires early surgical correction to prevent pulmonary hypertension
