  • may or may not be symptomatic

  • often associated with coarctation of the aorta

  • the more severe the narrowing, the earlier the presentation

  • higher risk of stenosis, endocarditis, dissecting aneurysms in adulthood

  • aortic valve is bicuspid in 50% of aortic valve coarctations

gross specimen: bicuspid aortic valve and LV hypertrophy

  • there are two cusps instead of three

  • the valve is also very narrowed

  • downstream, post-stenotic dilatation

  • upstream, LV hypertrophy

  • endocardial fibroelastosis is seen in left ventricle
