this is the only complex congenital heart disease to be discussed today
all four chambers are communicating with each other
endocardial cushions (EC) responsible for closure of the membranous ventricular septum and formation of mitral and tricuspid valve septal leaflets do not form properly
failure of EC leads to primum ASD, membranous VSD, and dysplastic mitral/tricuspid valves
33% occur in trisomy 21
gross specimen: AV canal
bottom half of the hole is the membranous VSD
top half of the hole is the primum type ASD
this is just a great big hole and there is very ineffective valve function
systemic pressure is once again applied to pulmonary vasculature
