Atrioventricular septal defect, aka AV canal or AV communis

  • this is the only complex congenital heart disease to be discussed today

  • all four chambers are communicating with each other

  • endocardial cushions (EC) responsible for closure of the membranous ventricular septum and formation of mitral and tricuspid valve septal leaflets do not form properly

  • failure of EC leads to primum ASD, membranous VSD, and dysplastic mitral/tricuspid valves

  • 33% occur in trisomy 21

gross specimen: AV canal

  • bottom half of the hole is the membranous VSD

  • top half of the hole is the primum type ASD

  • this is just a great big hole and there is very ineffective valve function

  • systemic pressure is once again applied to pulmonary vasculature
