  • autoimmune disease where part of TSH receptor is an autoantigen

  • fragments of TSH-R are present on thyroid and T cell MHC complexes

  • then, T cells reacting to this antigen stimulate B cells and plasma cells to produce autoantibodies to TSH-R

  • risk factors

    • genetic: HLA-DR3, CTLA-4, gender

    • environmental: stress, smoking, high iodine intake, viral infection


thyroid-stimulating immunoglobulin is a good diagnostic tool

  • this helps distinguish between, say, toxic multinodular goiter and Graves disease
