1. age: most occur in 20-49 yr olds (pts with pen allergies at 10 yrs old may not still be allergic later in life, but don’t take that risk)
2. previous rxn to penicillin: increases risk. 1-2 mos after a rxn, antibodies detectable in > 80%, declining steadily to less than 20% of pts by 10 yrs.
3. Type of previous rxn: anaphylaxis – 15% chance of re-reaction > urticaria – 8% > exanthem – 2% > other/uncertain – 0.5% > no rxn – 0.45%
4. General atopy (ie. pollen, allergies to normal things): NOT a risk factor for pen allergy
5. Route: parenteral pen is more provocative than oral
