Classification of Immune Reactions to Penicillin

*Type I: IgE on mast cells combines with penicillin antigen:

Immediate (< 1h) urticaria (hives), anaphylaxis (bronchospasm, hypotension,

cardiovascular collapse).

Accelerated (1-72h) usually urticaria

Type II: cytotoxic antibody, e.g. Coombs positive hemolytic anemia

Late reactions, >72h

Type III: immune complex (serum sickness, late reaction)

Type IV: cell mediated (contact dermatitis, late reaction)


*Type I is the type that we worry about, could be life threatening
